You are requesting information to re-register the shares on a custodial account where the custodian is now deceased. Depending on the age of the minor, you will need to follow the appropriate instructions for re-registration. Please note if the minor is over fourteen, you can use either set of instructions.
- Obtain a court-certified copy of the certificate of appointment of the Successor Custodian, dated within 60 days of the date we receive the request for transfer.
- Certified Death Certificate, or a photocopy affixed with a Medallion Signature Guarantee.
- Complete the reverse side of the stock certificate(s) or a stock power indicating the Successor Custodian's name, the minor's name, the appropriate address and social security number of the minor.
- The Successor Custodian must sign the reverse side of the original stock certificate or stock power and have the signature guaranteed by a participant in an approved Medallion Signature Guarantee program, which can be obtained at most commercial banks and brokerage firms. Notary stamps will not be accepted.
- The minor must write a letter of designation of the Successor Custodian signed by the minor which must be notarized or guaranteed with a medallion stamp.
- This appointed Custodian must write a letter accepting the appointed Custodianship.
- Certified Birth Certificate, or a photocopy affixed with a Medallion Signature Guarantee.
- Complete items 2, 3 & 4 from the list above.
Return the above documents to our office. For your protection, we suggest sending stock certificates, and accompanying documents, by certified mail to our physical address: Disney Shareholder Services, 611 N Brand Blvd, Suite 6100, Glendale CA 91203.
Once the transfer is complete, the shares will be held electronically by Disney's Direct Registration Service (DRS), unless a stock certificate is requested. If you have any questions regarding the above
